Letter of nomination(s) should be a minimum of 1 page, double-spaced in length. Chapter members, Council or Chapter Advisor can submit. Multiple nomination letters can be submitted.

Preferably only person in the photo. Should this nominee win the award, this photo will be used during the awards ceremony and social media.
